Required Physical Forms
Physical Form (Brand new physical forms for 2024-2025 from the State of NJ)
All athletes must complete to participate in a sport. This includes intramural and interscholastic sports. A parent/guardian must complete page 1, 2 and 3 and a signature is required. Your family doctor (or an Urgent Care/Med Express) must complete page 4 and 5 of the packet. Then return page 5 ONLY to the school nurse. Once it is returned, it may take up to a week for the school doctor to approve your physical, and then your child is allowed to participate in athletics.
Parents must complete with their digital signature prior to each season (Fall, Winter, Spring) in the Genesis Parent Portal.
